Forms Basics

Once you have found a form that you wish to fill out,
  1. Click on the first entry in the form with your mouse.

  2. Key or select the information requested.

  3. Hit the [Tab] key to go to the next text entry.
    or
    click over any entry with mouse to jump around.

  4. Repeat steps 2. and 3. until you get to the end of the form.

  5. Select the appropriate form button for the next action you want.

Common Form Problems

  1. If you you are using the same form(s) several times is succession and browser either
    • freezes or
    • gives you an error message or
    • obviously invalid information

    The problem may be cause by your browser's cache settings. This happens when your browser keeps old copies of the web pages in memory while you requesting newer pages.
    To change cache settings
    Netscape Browsers Microsoft Browsers
    click on
    1. Options menu
    2. Network Preferences menu
    3. Cache tab
    4. Once per Session next to "Verify Documents:"
    5. OK button at the bottom closes the screen.
    click on
    1. View menu
    2. Internet Options menu
    3. Settings button in "Temporary Internet files" box
    4. Every Time you start Internet Explorer under "Check for newer versions of stored pages"
    5. OK button at the bottom closes the window.
    6. Apply button at the bottom closes the screen.
    Sometimes it helps to clear the memory cache and/or disk cache in your browser too.
    To clear caches
    click on
    1. Options menu
    2. Network Preferences menu
    3. Cache tab
    4. Clear Memory Cache Now button
    5. Clear Disk Cache Now button
    6. OK button at the bottom closes the screen.
    click on
    1. View menu
    2. Internet Options menu
    3. Delete Files button in "Temporary Internet files" box
    4. OK button in "Delete Files" window.
    5. OK button at the bottom closes the screen.

    This problem may be also caused by mixing the use of browser buttons with form buttons. Examples occur after pressing a form button and while still waiting for the next page to finish loading

    • Reload or Refresh buttons or menu items
    • Back or Forward buttons or Go menu items

    Simply put,
    • Each screen is like a locked door, and the buttons are the keys that open that door.
    • Each screen has a different lock, that requires a different set of key(s).
    • The current set of keys (buttons) can only open the current door (screen).
    • The key has to match the lock in order to get to the next key or screen.
    • Using the browser buttons to move to another screen means the key and lock are nolonger matched.
    • Thus they for will not work.

  2. Push Button that says "dm" or pull down option is "dm"

    "dm" stands for "Data Missing". It is caused by an Internet transmission error. The button or data item will function correctly. Do Not "Reload" the form or it will probably malfunction. If you feel uncertain how to proceed, try restarting with first form.
